Subsection (a) of section 6411 adds a new clause (xi) to section 3351(a)(2)(A) of the Public Health Service Act. For each of fiscal years 2026 through 2040 the amount is the previous year's amount multiplied by 1.07, then multiplied by the ratio of the number of people enrolled in the program on July 1 of that previous year to the number enrolled on July 1 of the year before that.

Subsection (b) of section 6411 requires the Secretary of Health and Human Services, within three years of enactment, to assess the program's expected budget authority and outlays through its duration and report to the Speaker and minority leader of the House, the majority and minority leaders of the Senate, and four named committees. The report must project needs year by year through fiscal year 2090, review the program's modeling for 2017 onward against actual spending, compare projections under the new formula with projections if it ran through 2090, and give any recommendations for changing the formula.
