People already enrolled get 18 months and a review
What the document says“Not later than 1 year after the date of the enactment of this section, the Commissioner of Social Security shall complete a review of individuals entitled to, or enrolled for, benefits under this title as of such date of enactment”
The section applies the new limit to people already entitled or enrolled at enactment only from 18 months after that date, and requires the Commissioner of Social Security within one year to review those people to identify anyone not within the four statuses, and to notify each such person that their entitlement or enrollment will end 18 months after enactment, as soon as practicable and in a way designed to make sure they understand.

The social security head must review everyone on Medicare. That must be done within a year. The clock starts with this law.
The review looks for people outside the four groups. Each is told their cover will end. Cover ends 18 months after this law.
